Summary

Hartford HealthCare is seeking a dedicated and compassionate Care Coordinator (LMSW) to join our Case Coordination team. As a Care Coordinator, you will have the opportunity to make a difference in the lives of patients and help ensure they receive the best possible care. We are looking for someone with a Master's in Social Work and a minimum of 3 years of Medical Social Work experience. If you are passionate about healthcare and enjoy working in a collaborative environment, this role is for you.

Role and Responsibilities

• Collaborate with patients, nurses, social workers, physicians, and healthcare team members to coordinate and optimize care
• Communicate effectively with patients and their families to provide support and ensure their needs are met
• Utilize Microsoft Office, Outlook, Epic software, Morrissey, and web-based programs effectively
• Facilitate care along a continuum through effective resource coordination

Qualifications

• Master's in Social Work from an accredited school of Social Work
• Minimum of 3 years of Medical Social Work experience, preferably in acute care
• Current license as a Master's in Social Work in the State of CT (LMSW)
• Case Management certification ACM or CCM strongly preferred
• Proficient in Microsoft Office, Outlook, and Epic software
