Responsibilities include:

• Collaborating with patients, nurses, social workers, physicians, and the healthcare team to provide optimal care management
• Facilitating care coordination and effective resource utilization
• Promoting patient self-determination
• Utilizing electronic medical records and relevant software programs

Qualifications:

• Master's degree in Social Work from an accredited school of Social Work
• Minimum of 3 years of Medical Social Work experience (Acute Care experience preferred)
• Current license as a Licensed Master of Social Work (LMSW) in the State of Connecticut
• Case Management certification (ACM or CCM) strongly preferred
• Proficient in Microsoft Office, Outlook, Epic software, Morrissey, and web-based programs
