Responsibilities

  • Provide individual, couple, group, and family therapy based on each client's specific needs
  • Prepare accurate and comprehensive medical documentation
  • Assess and plan care by evaluating the psychiatric and psychosocial needs of clients
  • Deliver in-home clinical services and crisis intervention
  • Educate clients and family members on available resources and provide support
  • Collaborate with internal and external partners for integrated healthcare delivery
  • Participate in administrative activities, team meetings, and staff education

Requirements

  • Master's Degree in Social Work, Counseling, Psychology, or related field
  • Active CT License in Social Work, Professional Counseling, Alcohol & Drug Counselor, Marital & Family Therapy
  • Minimum of three years of experience in psychiatric and/or substance abuse population
  • Proficiency in assessment, diagnosis, and individual/group therapy
  • Computer literacy and basic keyboarding skills
