Job Description:
As a Behavioral Health Clinician, you will provide quality and cost-effective inpatient/outpatient and/or community care for clients and families in a target population across the continuum of care. You will work in coordination with interdisciplinary teams to organize, prepare, and deliver evidence-based clinical services reflected by a multi-disciplinary plan of care.
Responsibilities:
- Evaluate the psychiatric and psychosocial needs of patients/clients and establish a multidisciplinary plan to address identified issues and goals.
- Utilize individual, couple, group, and family therapy to address each patient/client's needs as part of the multidisciplinary treatment team.
- Provide client medical documentation based on program needs, including social history, treatment plans, progress notes, and discharge summaries.
- Coordinate and implement client/patient/family care along the continuum by collaborating with multidisciplinary teams, payers, and community/regulatory agencies.
- Educate patients/clients and family members, ensuring seamless, coordinated transitions across levels of care.
- Provide direct patient/client care, including individual and group therapy, advocacy, referral, and discharge planning.
- Regularly evaluate the effectiveness and efficiency of care plans, documenting patient/family responses and revising plans as needed.
- Negotiate level of care and length of stay with third-party payers.
- Participate in administrative and development activities, including team meetings, supervision of staff, and staff education.
Qualifications:
- Master's Degree in Social Work, Counseling, or a related field.
- Three years of experience providing direct services to psychiatric patients and substance use population.
- Previous experience in assessment, diagnosis, individual/group therapy, and crisis intervention.
- Active CT License in Social Work, Professional Counseling, Alcohol & Drug Counselor, or Marital & Family Therapy.
