Summary

Hartford HealthCare is looking for a dedicated and compassionate Behavioral Health Clinician to join our team at Glastonbury Adult Intensive Outpatient Program. As a Behavioral Health Clinician, you will play a crucial role in providing inpatient/outpatient and community care to clients and families across the continuum of care. We are seeking candidates with a Master's Degree in Social Work, Counseling, or a related field, along with at least three years of experience providing direct services to psychiatric patients and the substance use population. Responsibilities

  • Provide direct client care using individual, couple, group, and family therapy specific to each client's needs as indicated on the treatment plan.
  • Prepare appropriate medical documentation to meet regulatory requirements of licensing and third party agencies as well as internal policies.
  • Assess and plan care by evaluating the psychiatric and psychosocial needs of the client; organize and prioritize care and treatment and advocate appropriate resources for the client and family.
  • Provide in-home clinical services and/or direct crisis intervention to clients and families in multiple sites in the system and in the community.
  • Provide continuous education to client and family members and refer them to appropriate levels of care, support groups, and other available resources to maximize care and facilitate the recovery or treatment process.
  • Proficient in working effectively across programs for clients with both mental health and substance use disorders.
  • Negotiate level of care and length of stay with managed care companies; continually re-evaluate client's needs and inform the third party payers of ongoing changes in the client's status.
  • Work collaboratively with internal and external partners along the continuum of care to provide integrated healthcare delivery to all clients and families.
  • Conduct emergency evaluations for clients in the community if required by the program.
  • Participate in administrative activities including team meetings, clinical supervision of staff, and staff education as designed by the Program Manager.
  • Assume responsibility for self-improvement by identifying, communicating, and seeking resources to meet own learning needs.
  • Perform other related duties as required or assigned by the Program Manager.

Qualifications

  • Master's Degree in Social Work, Counseling, or a related field
  • Three years' experience providing direct services to psychiatric patients and the substance use population
  • Previous experience in assessment and diagnosis, individual/group therapy, and crisis intervention
  • Active CT License in Social Work, Professional Counseling, Alcohol & Drug Counselor, Marital & Family Therapy
