Job Description

As a Behavioral Health Clinician, you will provide quality, cost-effective care for clients and families in the Behavioral Health Home program. You will work in coordination with interdisciplinary teams to organize, prepare, and deliver evidence-based clinical services across the continuum of care. Your responsibilities will include:

  • Evaluating the psychiatric and psychosocial needs of patients to assess and plan care
  • Utilizing individual, couple, group, and family therapy to address each patient's needs
  • Providing client medical documentation based on program needs
  • Coordinating and implementing client care along the continuum
  • Educating patients and family members
  • Evaluating the effectiveness and efficiency of the plan of care

Qualifications

To be successful in this role, you should have:

  • Master's Degree in Social Work, Counseling, or Related Field
  • Three years' experience providing direct services to psychiatric patients and substance use population
  • Previous experience in assessment, diagnosis, individual/group therapy, and crisis intervention
  • Active CT License in Social Work, Professional Counseling, Alcohol & Drug Counselor, Marital & Family Therapy
