Responsibilities

  • Evaluate the psychiatric and psychosocial needs of clients and develop a multidisciplinary plan of care
  • Conduct individual, couple, group, and family therapy in accordance with the treatment plan
  • Coordinate and implement client care across the continuum by collaborating with the multidisciplinary team
  • Educate clients and family members and ensure seamless transitions across levels of care
  • Provide direct patient/client care, including therapy, advocacy, referral, and discharge planning
  • Regularly evaluate the effectiveness of care and revise the plan as needed
  • Work across programs for clients with mental health and substance use disorders

Qualifications

To qualify for this role, you will need:

  • Master's Degree in Social Work, Counseling, or a related field
  • Three years of experience providing direct services to psychiatric patients and substance use population
  • Previous experience in assessment, diagnosis, and individual/group therapy
  • Active CT License in Social Work, Professional Counseling, Alcohol & Drug Counselor, Marital & Family Therapy
