As a Nurse Practitioner or Physician Assistant, you will be an integral part of our REACH (Resource for Elderly And Care at Home) Program. Your role will involve working within a multi-disciplinary care team to deliver high-quality healthcare services to geriatric patients in their homes.

Qualifications

To be considered for this position, you must meet the following requirements:

• Unrestricted State of Connecticut Medical License (or eligible)
• APRN (Family, Adult/Geri Nurse Practitioner) or PA
• At least two years of post-graduate experience as a Nurse Practitioner or Physician Assistant
