Summary:

Hartford HealthCare is seeking a Clinical Consultant for the Meriden Court. In this role, you will be responsible for providing screening and risk assessment to individuals waiting for arraignment to screen for behavioral health conditions and provide consultation and collaboration to court personnel in regards to recommended treatment. Job Responsibilities:

  • Provide direct client care using individual, couple, group, and family therapy specific to each client’s needs as indicated on the treatment plan.
  • Prepare appropriate medical documentation to meet regulatory requirements of licensing and third-party agencies.
  • Assess and plan care by evaluating the psychiatric and psychosocial needs of the client, organize and prioritize care and treatment, and advocate appropriate resources for the client and family.
  • Provide direct crisis intervention to clients and families in multiple sites in the system and in the community.
  • Provide continuous education to clients and family members and refer them to appropriate levels of care, support groups, and other available resources to maximize care and facilitate the recovery or treatment process.
  • Proficient in working effectively across programs for clients with both mental health and substance use disorders.
  • Negotiate level of care and length of stay with managed care companies, continually re-evaluate client’s needs, and inform the third-party payers of ongoing changes in the client’s status.
  • Work collaboratively as a team member along the continuum of care to provide an integrated delivery of healthcare to all clients and families.
  • Participate in administrative activities including team meetings, clinical supervision of staff, and staff education as designed by the Program Manager.
  • Assume responsibility for self-improvement by identifying, communicating, and seeking resources to meet own learning needs.

Qualifications:

  • Master's Degree in Social Work, Counseling, or related field.
  • Three years' experience providing direct services to psychiatric patients and/or substance abuse population.
  • Previous experience in assessment and diagnosis and individual and group therapy.
  • Valid CT Licensed Masters of Social Work if Master's degree is in Social Work.
  • Works towards obtaining licensure requirements.
  • Demonstrated experience using a computer; basic keyboarding skills.
  • Flexible with schedule per program needs, including evenings/weekends and travel between sites across the organization.
